When you have reason to believe that someone in your employment may be under influence of Drugs and or Alcohol, you should use this form to observe and record the behaviour.
This is a first step in the process of following up on suspected substance use impacting employee performance.
The Observation Form should be retained on the employee’s employment file and form part of your Drug & Alcohol Policy.
